Confirmation Preparation
Welcome to the SJN Confirmation Program for Teens. In general, we begin preparing for Confirmation in the spring of 8th grade for all teens who are eligible for and desire to receive the Sacrament of Confirmation. The Sacrament is then celebrated in the fall of 9th grade. (Some teens prepare for and receive the Sacrament in a later grade.)
The information below is for teens who are eligible and have been invited by email to our Confirmation Prep Program. The parents of all eligible teens will be sent information for the upcoming Confirmation Preparation Program by email on April 10th (2026). Please contact Angela Davis or Eric Munoz if you do not receive this information or have any questions.

Commitment Form
Parents should complete this online form by April 30. There is a $20 fee with the registration to help with the cost of robes, snacks, and other materials/supplies. Please let us know if you would like to request a scholarship.
Saint Name Form
Small Group sessions will talk some about choosing a Saint name. Youth should complete this online form by June 11.
Sponsor Certificate of Eligibility
Choosing a Sponsor if very important. Sponsors must be:
- A practicing Catholic who has received the Sacrament of Confirmation
- At least 16 years old
- Someone who can assist you in growing in faith
- Someone other than your parent (parents cannot be Confirmation Sponsors for their own children)
If your Sponsor is NOT a registered member of St. John Neumann church, then they must obtain a Certificate of Eligibility from their parish. We do not have blank forms for other churches to complete. The Sponsor just needs to contact the main office of their parish. Sometimes this process take time, so please ask that your sponsor begins this process as soon as possible.
Forms can be mailed to SJN or scanned and EMAILED TO BETSY!
